Impact of Screening Threshold of Quantitative Fecal Immunochemical Test on Results of Early Screening for Colorectal Tumors

Abstract
Objective To explore the impact of the screening threshold of quantitative fecal immunochemi-cal test(qFIT)on the results of early screening for colorectal tumors.Methods Participants who underwent qFIT and colonoscopy in our hospital from March 2024 to February 2025 were enrolled.The qFIT results of people with different types of colorectal tumors and healthy people were compared,the correlation between qFIT results and tumor diagnosis results was analyzed,and the diagnostic efficacy of different screening thresh-olds for advanced colorectal tumors was assessed.Results A total of 410 cases(4.44%)were qFIT posi-tive,among which 90 cases received colonoscopy subsequently,with a compliance rate of 21.95%.The qFIT value of patients with colorectal cancer was significantly higher than that of patients with advanced adenoma and those with other colonoscopy diagnosis results(P<0.05).Multivariate analysis showed that the qFIT value≥100 μg/L was an independent risk factor for the occurrence of advanced colorectal tumors(P<0.05).The area under the receiver operating characteristic curve(AUC)of qFIT for diagnosing advanced colorectal tumors was 0.798(0.688～0.863).When the positive threshold was increased from 100 μg/L to 500 μg/L,the sensitivity of qFIT for diagnosing advanced colorectal tumors decreased from(94.5±2.8)%(85.4%～99.1%)to(50.8±1.5)%(39.3%～63.2%),and the specificity increased from(27.3±0.8)%(24.3%～31.2%)to(74.7±2.2)%(70.6%～78.9%).Conclusion The qFIT value≥100 μg/L is an independent risk factor for advanced colorectal tumors.Using 100 μg/L as the screening threshold for diagnosing advanced colorectal tumors can obtain high sensitivity,which is suitable for early screening of physical examination population and has certain clinical popularization and application value.关键词
