Introduction of YANG Peiqun's Experience in Treating Stroke-Associated Pneumonia Complicated by Multidrug-Resistant Bacterial Infection Using Method of Expelling Heat from Ying to Qi

Abstract
Stroke-associated pneumonia(SAP)complicated by multidrug-resistant(MDR)bacterial infection commonly occurs in elderly stroke patients with multiple comorbidities and long-term bed rest.It is clinically characterized by polymicrobial infection,poor response to broad-spectrum antibiotics,intense inflammatory response,and a tendency to progress to sepsis,belonging to a critical and refractory condition.Professor YANG Peiqun proposes that its core pathogenesis involves the inter-binding of toxin,stasis,phlegm,and heat deeply lodged in the ying and blood,resulting in the obstruction of qi movement.The therapeutic principles are"expelling heat outward"and"turning qi inward."He emphasizes clearing the yingfen to expel heat while simultaneously activating the stagnated qi movement.In practice,Maxing Shigan Decoction combined with Qianjin Weijing Decoction is often used to expel pathogens from the weifen and qifen,accompanied by Shengjiang Powder to pivot the qi movement in the ying and blood,jointly achieving the effect of expelling heat from ying to qi.This article systematically summarizes Professor YANG's theoretical basis,prescription strategies,and key points of clinical modification in treating SAP with the above method,supported by critical case records.It reflects the therapeutic advantage of traditional Chinese medicine in multi-target and multi-level regulation of complex infectious diseases,providing a reference for the integrated traditional Chinese and western medicine treatment of SAP complicated by MDR infection.关键词
