价值共创视角下"多长高转"重点患者管理提升机制研究:基于某三甲医院的分析
Research on the Mechanisms to Enhance Key Patient Management of"Multi-Morbidity,Long-Stay,High-Cost,and Transfer"from the Perspective of Value Co-Creation Based on Analysis of A Tertiary Hospital

Abstract
Objective To explore the systematic management pathways for"Multi-morbidity,Long-stay,High-cost,and Transfer"(MLHT)patients from the perspective of value co-creation,and construct mechanisms for improving medical quality and safety adapted to the characteristics of complex,high-risk patient groups.Methods Through policy document analysis and do-mestic and international literature review,using the management practices of a tertiary hospital as a case study,the full-process governance mechanism was analyzed from three dimensions of stakeholder collaboration,professional integration,and information technology empowerment.Results By establishing a value co-creation management system supported by hierarchical control,mul-tidisciplinary collaboration,and closed-loop information,the hospital achieved significant results,such as reducing complication from 0.31%to 0.20%,low-risk mortality rate from 0.0054%to 0.0046%,decreasing new disputes by 75%,valid complaint by 24%,and increasing no-fault case by 20%.Conclusion A value co-creation-oriented governance model for MLHT patients can effectively integrate multiple stakeholders and resources,promote a shift in complex patient management from passive response to active collaboration,and provide a replicable practical paradigm for continuous improvement of medical quality and high-quali-ty hospital development.关键词
