An Empirical Study on the Impact of DRG Payment Reform on Hospital Medical Service Performance of Frontier Regions Based on Interrupted Time Series Analysis

Abstract
Objective To evaluate the impact of Diagnosis-Related Group(DRG)payment reform on medical service per-formance in a tertiary hospital in Xinjiang,and provide evidence for its implementation in border region hospitals.Methods A ret-rospective interrupted time-series analysis was conducted to analyze the monthly data(Jan 2019-Dec 2024)in this hospital.An indicator system covering service capacity,resource efficiency,patient safety,and cost containment was constructed to assess the reform's influence.Segmented regression model was used to analyze the policy's effects.Results After the reform,the average per hospitalization cost decreased significantly(β3=-0.0132,P=0.0092),and the mortality rate in the medium-and low-risk groups declined(β2=-0.1061,P=0.0391).However,the Case Mix Index(CMI)showed a short-term decrease post-reform(β2=-0.0697,P=0.0068),indicating the need for hospitals to optimize their case mix structure to mitigate the risk of reduction in case complexity.Conclusion The DRG reform was effective in controlling costs and improving safety,but posed chal-lenges in enhancing resource efficiency.Hospitals should optimize case structures and processes,implement DRG-based clinical pathways,and leverage medical alliances to balance cost control with high-quality development.关键词
