儿童结肠镜检查前肠道准备质量影响因素的横断面调查
A cross-sectional survey on influencing factors of intestinal preparation quality before colonoscopy in children

Abstract
Background Colonoscopy is a crucial diagnostic tool for pediatric gastrointestinal diseases,and its effectiveness heavily relies on the quality of bowel preparation.Currently,the rate of inadequate bowel preparation in children is high relatively,affecting the safety and efficiency of diagnosis and treatment directly.The relevant influencing factors still need to be identified systematically.Objective To explore the key factors influencing the quality of bowel preparation before colonoscopy in children,and provide evidence for optimizing the clinical preparation process.Design Cross-sectional study.Methods A total of 237 hospitalized pediatric patients who underwent colonoscopy in the Gastroenterology Department of Shenzhen Children's Hospital from January to July 2024 were selected as the study subjects.Data on gender,age,last bowel movement effect,and the interval between laxative administration and examination were collected through a questionnaire survey.The quality of bowel preparation was assessed using the Boston bowel preparation scale(BBPS).Logistic regression analysis was employed to analyze influencing factors,and the Goodman-Kruskal Gamma test was used to evaluate the correlation between variables and BBPS.Main Outcome Measures Intestinal preparation quality(BBPS score).Results The response rate of the survey respondents to the questionnaire survey was 98.34%(237/241).Among the 237 pediatric patients,the non-conformity rate of bowel preparation was 25.7%(61/237).Among them,gender(β=-0.784,P=0.006)and the effect of the last bowel movement(β=2.421,P<0.001)were independent influencing factors,and the highest conformity rate(94.7%)was achieved when the examination was conducted 3 to 4 hours after the administration of laxatives.Conclusion The study confirms in the domestic pediatric population for the first time that the child's gender,the effectiveness of the last bowel movement,and the time interval between the administration of laxatives and the examination are key factors affecting the quality of bowel preparation systematically.It provides a reference basis for strengthening personalized behavioral guidance for male pediatric patients,dynamic monitoring of the quality of the last bowel movement,and time control from medication administration to examination during the bowel preparation process.关键词
