Abstract
Objective In order to explore the relationship between serum caveolin-1(Cav-1)and CXC chemokine ligand 1(CXCL1)levels with the severity and prognosis of psoriasis vulgaris patients.Methods Totally 150 patients with psoriasis vulgaris in Taiyuan central hospital were labeled as the study group and assigned into mild,moderate,and severe groups according to the severity of disease.They were also assigned into good prognosis group and poor prognosis group based on the psoriasis area and severity index(PASI)at the sixth months after treatment.Clinical data were collected,serum levels of Cav-1 and CXCL1 were detected,and their correlation with clinical indicators were analyzed.Multivariate logistic regression analysis was performed to identify prognostic risk factors.The receiver operating characteristic(ROC)curve was used to evaluate the prognostic efficacy of serum Cav-1 and CXCL1,and internal validation of the model was conducted.Results Compared with the mild group,serum Cav-1 level in the moderate and severe groups was obviously lower(P<0.05),and CXCL1 level was obviously higher(P<0.05).Moreover,serum Cav-1 level in the severe group was significantly lower than that in the moderate group(P<0.05),and CXCL1 level was significantly higher than that in the moderate group(P<0.05).The poor prognosis group had significantly lower serum Cav-1 level than the good prognosis group(P<0.05),and obviously higher CXCL1 level than the good prognosis group(P<0.05).White blood cell count(WBC),neutrophil ratio,C-reactive protein(CRP),erythrocyte sedimentation rate(ESR),and PASI score were negatively correlated with serum Cav-1 level(P<0.05),and clearly positively correlated with CXCL1 level(P<0.05).Decreased serum Cav-1 level,increased CXCL1 level,and ESR were risk factors affecting the prognosis of these patients(P<0.05).The risk of poor prognosis in patients with low level of serum Cav-1 and high level of CXCL1 was 6.295 times higher than that with high level of Cav-1 and low level of CXCL1(baseline risk),and the risk caused by the interaction between the two accounted for 25.45%.The area under the curve(AUC)values of serum Cav-1 and CXCL1 alone and in combination in evaluating prognosis were 0.805,0.803 and 0.910,respectively,with the combination being superior to the individual evaluations(Zcombination-Cav-1=2.783,P=0.005,Zcombination-CXCL1=2.446,P=0.014),and internal validation showed good model consistency(P=0.302).Conclusion Serum Cav-1 level is decreased and CXCL1 level is increased in patients with psoriasis vulgaris.They are related to the severity of the disease and prognostic outcomes,and can be used as indicators to evaluate prognosis.In addition,the joint evaluation of the two is more effective.关键词
