Abstract
Objective To analyze the current status and influencing factor of cancer pain management disorder in elderly patients with advanced colorectal cancer.Method One hundred and two elderly patients with advanced colorectal cancer admitted to Beijing Hospital of Traditional Chinese Medicine Affiliated to Capital Medical University from June 2021 to June 2024 were enrolled for the research.The Chinese version of barriers questionnaire(BQ)was performed to evaluate the level of cancer pain management disorder in all patients.The total score of BQ scale was compared among patients with different characteristics.The median of the total score of the BQ scale for elderly patients with advanced colorectal cancer was used as the cutoff value.One hundred and two patients were equally divided into the high BQ score group(51 cases,more severe barriers in cancer pain management)and the low BQ score group(51 cases,less severe barriers in cancer pain management),which were used as dependent variables,Multiple Logistic regression analysis was performed to explore the influencing factor of the high score of BQ scale.Pearson correlation analysis was used to explore the correlation between the total score of BQ scale and self-perceived burden scale for cancer patients scale(SPBS-CP),Herth hope index(HHI)scale.Result The total score of BQ scale for all patients was(107.12±23.98)points,with an average score of(3.15±0.71)points for each item.The total score of the BQ scale for patients aged≥70 years old,no receiving educational activities related pain management,tumor stage Ⅳ were significantly higher than that of patients aged<70 years old,receiving educational activities related pain management,tumor stage ⅢB(all P<0.05).There were statistically significant differences in the total score of BQ scale for patients with different educational levels,per capita monthly income of families,self-perceived burden and hope level(all P<0.05).As the educational level decreased,per capita monthly income of families decreased,self-perceived burden increased,and hope level decreased,the total score of BQ scale gradually increased.Multivariate Logistic regression analysis showed that college education or above,receiving educational activities related pain management and high hope level were the independent protective factors for high score of BQ scale in elderly patients with advanced colorectal cancer(all P<0.05),and moderate to severe self-perceived burden was the independent risk factor(P<0.05).Pearson correlation analysis results showed that the total score of BQ scale in elderly patients with advanced colorectal cancer was positively correlated with total score of SPBS-CP scale(P<0.05),and negatively correlated with the total score of HHI scale(P<0.05).Conclusion The level of cancer pain management disorder in elderly patients with advanced colorectal cancer is not high.Education level,educational activities related pain management,self-perceived burden and hope level were the independent influencing factors of cancer pain management disorder.Early targeted interventions should be emphasized in clinical practice.关键词
