拮抗针法联合头针治疗卒中后肩手综合征的临床研究
Clinical Study on Antagonistic Needling Combined with Scalp Acupuncture in Treatment of Post-Stroke Shoulder-Hand Syndrome

Abstract
Objective:To investigate the clinical efficacy of antagonistic needling combined with scalp acupuncture in treatment of post-stroke shoulder-hand syndrome.Methods:A total of 76 patients with post-stroke shoulder-hand syndrome were randomly assigned into the control group and the observation group,with 38 patients in each group.The control group was treated with conventional acupuncture;on which basis,the observation group was also treated with antagonistic needling combined with scalp acupuncture.After the treatment,the two groups were compared in terms of short-form McGill pain questionnaire including pain rating index(PRI),visual analogue scale(VAS),present pain intensity(PPI),disabilities of the arm,shoulder and hand(DASH)questionnaire,Fugl-Meyer assessment for upper limb motor function,edema degree score,shoulder joint range of motion,grip strength,clinical efficacy and adverse reactions.Results:After the treatment,the observation group showed significantly lower scores of PRI,VAS,PPI,edema severity,and DASH,and significantly higher Fugl-Meyer score than the control group(P<0.05).Both groups demonstrated significant improvements in shoulder flexion,extension,abduction,and grip strength after the treatment;however,these improvements were more significant in the observation group(P<0.05).The total effective rate was 92.11%(35/38)in the observation group,which was significantly higher than 73.68%(28/38)in the control group(P<0.05).There was no significant difference in the incidence of adverse events between the two groups(P>0.05).Conclusion:Antagonistic needling combined with scalp acupuncture is an effective treatment for post-stroke shoulder-hand syndrome,which can significantly relieve pain,reduce edema,and improve upper limb function.关键词
