Correlation of abdominal fat distribution and muscle content with non-alcoholic fatty liver disease in people with obesity

Abstract
Objective:To investigate the correlation and diagnostic value of abdominal fat distribution characteristics and muscle content on non-alcoholic fatty liver disease(NAFLD)in obese people by computed tomography(CT).Methods:This cross-sectional study enrolled 226 obese subjects.Abdominal adipose tissue parameters,including abdominal fat volume,abdominal cavity volume,visceral fat area,subcutaneous fat area,total fat area and visceral-to-subcutaneous fat area ratio(V/S),as well as skeletal muscle parameters,including area of psoas major muscle,area of erector muscle,area of quadratus lumborum muscle,area of transversus abdominis muscle,area of internal oblique muscle,area of external oblique muscle,area of rectus abdominis muscle,total skeletal muscle area and skeletal muscle index were quantitatively analyzed using axial abdominal CT scans.Participants were divided into a normal group and a fatty liver group based on the liver-to-spleen CT attenuation ratio(CTL/S).To analyze the differences between the two groups,the correlation between each index and CTL/S,and to draw Receiver Operating Characteristic curve(ROC)to analyze and compare the diagnostic value of different models for NAFLD.Results:Significant differences in abdominal fat distribution and skeletal muscle parameters were observed between the NAFLD group and the normal group(P<0.05).All indices exhibited significant negative correlations with the CTL/S(P<0.05).The combined fat-muscle indicator diagnostic model demonstrated significantly superior performance(AUC=0.790)compared to the single fat parameter model(AUC=0.705)and the combined fat-traditional weight indicator model(AUC=0.711),with statistically significant differences(P<0.05).Conclusion:The combined model incorporating CT-derived fat and muscle indices shows high diagnostic value for NAFLD and provides a new tool for the early diagnosis.关键词
