穴位贴敷联合益气通络健脾补肾疗法在慢性阻塞性肺疾病稳定期的应用效果及对生活质量的影响
Effects of acupoint patching combined with qi-tonifying,collateral-unblocking,spleen-strengthening and kidney-nourishing therapy on stable chronic obstructive pulmonary disease:clinical outcomes and quality of life

Abstract
Objective To evaluate the clinical effects of acupoint patching combined with a qi-tonifying,collateral-unblocking,spleen-strengthening and kidney-nourishing therapy in patients with stable chronic obstructive pulmonary disease(COPD),and to assess its impact on quality of life.Methods One hundred eighty patients with stable COPD were randomized into three groups(n=60 each)using a random number table.The patch group received acupoint patching;the herbal group received the qi-tonifying,collateral-unblocking,spleen-strengthening and kidney-nourishing therapy;the combined group received both interventions.All treatments continued for 9 months.Outcome measures before and after treatment included forced expiratory volume in 1 second(FEV1),forced vital capacity(FVC),FEV1/FVC ratio,primary and secondary clinical symptom scores,St.George's Respiratory Questionnaire(SGRQ)score,6-minute walk test(6MWT)distance,number of acute exacerbations,and oxidative stress markers(superoxide dismutase[SOD],glutathione peroxidase[GSH-Px],malondialdehyde[MDA]).Adverse events were recorded.Results All groups showed significant improvements from baseline in FEV1,FVC,and FEV1/FVC after treatment(P<0.05).Post-treatment values were highest in the combined group,followed by the herbal group,and lowest in the patch group(P<0.05).Primary and secondary symptom scores and SGRQ scores decreased significantly in all groups versus baseline(P<0.05),with the combined group showing the greatest reduction,followed by the herbal and patch secondary symptom scores and SGRQ scores decreased significantly in all groups versus baseline(P<0.05),with the combined group showing the greatest reduction,followed by the herbal and patch groups(P<0.05).6MWT distance increased significantly in all groups(P<0.05);post-treatment distance was greatest in the combined group,intermediate in the herbal group,and smallest in the patch group(P<0.05).The combined group experienced fewer acute exacerbations than the patch and herbal groups(P<0.05);there was no significant difference in exacerbation frequency between the patch and herbal groups(P>0.05).Markers of antioxidant capacity(SOD and GSH-Px)increased and MDA decreased in all groups after treatment(P<0.05).The combined group showed the largest favorable changes(SOD and GSH-Px:combined>herbal>patch;MDA:combined<herbal<patch;P<0.05).Overall adverse reaction rates did not differ significantly among groups(P>0.05).Conclusion Combining acupoint patching with qi-tonifying,collateral-unblocking,spleen-strengthening and kidney-nourishing therapy in patients with stable COPD yields greater improvements in pulmonary function,symptoms,exercise tolerance and quality of life,reduces acute exacerbation frequency and oxidative stress,and is well tolerated.关键词
