卡瑞利珠单抗联合SP化疗方案治疗局部晚期食管癌临床疗效及对细胞免疫、预后的影响
Clinical Curative Effect of Camrelizumab Combined with SP Chemotherapy in Advanced Esophageal Cancer and Its Influences on Cellular Immunity and Prognosis

Abstract
Objective To explore clinical curative effect of camrelizumab combined with S-1 plus cisplatin(SP)chemotherapy in advanced esophageal cancer and its influences on cellular immune function and prognosis.Methods A total of 202 patients with advanced esophageal cancer treated in The Second People's Hospital of Jiaozuo were enrolled between March 2021 and June 2024.According to random number table method,they were divided into control group(101 cases,SP chemotherapy regimen)and observation group(101 cases,camrelizumab combined with SP chemotherapy regimen).The clinical curative effect,serum tumor markers(CEA,CA19-9,CYFRA21-1,SCC),T lymphocyte subsets(CD3+,CD4+,CD8+)and the occurrence of adverse reactions were compared between the two groups.Results After treatment,objective response rate(ORR)and disease control rate(DCR)in observation group were and(P<0.05),higher than those in control group.The levels of CEA,CA19-9,CYFRA21-1 and SCC in observation group were(P<0.05),lower than those in control group The levels of peripheral blood CD3+,CD4+and CD4+/CD8+in observation group were higher than those in control group(P<0.05),while CD8+was lower than that in control group.The differences in all the above indexes between the two groups were statistically significant(P<0.05).In terms of safety,there was no significant difference in the incidence of adverse reactions between the two groups(P>0.05).After 1 year of follow-up,cumulative survival rate in observation group was higher than that in control group(Log-rank χ2=9.221,P=0.002).Conclusion The combination therapy of camrelizumab and SP regimen can significantly improve curative effect and survival status in patients with advanced esophageal cancer by significantly down-regulating tumor indexes and activating immune function.关键词
