老年肺癌化疗引起的周围神经病变患者跌倒恐惧的影响因素分析
Analysis of the influencing factors of fear of falling in elderly patients with peripheral neuropathy caused by chemotherapy for lung cancer

Abstract
Objective To analyze the influencing factors of fear of falling in elderly lang cancer patients with chemothera-py-induced peripheral neuropathy(CIPN).Methods A total of 150 elderly lung cancer patients with CIPN who were admit-ted to Jiujiang Third People's Hospital from January 2022 to December 2024 were selected.According to the score of the international falls risk assessment scale(FES-I),the patients were divided into the high fear group(≥23 points,n=89)and the low fear group(＜23 points,n=61).Through univariate analysis and logistic multirariate analysis,the influencing factors of fall fear in elderly lung cancer CIPN patients were identified.Results The single-factor analysis showed that there were statistically significant differences between the two groups in terms of age,CIPN CTCAE grade,number of comorbidities,Berg balance score,risk of home environment,and history of falls in the past 6 months(P＜0.05).The results of the multi-variate analysis showed that age≥60 years(β=1.322,OR=3.732,95%CI:1.623-8.753),CIPN≥2 grades(β=1.893,OR=6.114,95%CI:2.584-16.743),Berg balance score＜45(β=1.564,OR=4.835,95%CI:1.582-12.174),history of falls(β=1.483,OR=4.394,95%CI:1.101-12.321),and high-risk home environment(β=0.921,OR=2.516,95%CI:1.084-5.825)were independent influencing factors for fear of falling(P＜0.05).Conclusion Among elderly patients with lung cancer and CIPN,the incidence of fear of falling is relatively high.This occurrence is significantly associated with old age,severity of neurological impairment,balance dysfunction,history of falls,and environmental hazard factors.Clinically,multi-dimensional prevention measures should be implemented for high-risk individuals to improve the quality of life of these patients.关键词
