"双打包"支付方式下医患满意度的调查研究
Investigation on the satisfaction of physicians and patients under the"Double Package"payment

Abstract
Objective To analyze the current status and influencing factors of satisfaction among medical staff and patients under the"Double Package"medical insurance payment reform in Sanming City,and to provide evidence for improving the payment method reform.Methods In December 2024,a field survey was conducted in the leading hospitals of healthcare confederations and nine primary branches in Yong'an,Mingxi,and Datian counties(cities)of Sanming.A total of 269 valid questionnaires from medical staff and 315 from patients were collected.The questionnaires were revised based on expert consultations and demonstrated good reliability and validity.Descriptive statistics and multiple linear regression were used for data analysis.Results The overall satisfaction score of medical staff with the payment reform was 3.70±1.03,with a satisfaction rate of 87.5%.Satisfaction with technical skill improvement had the greatest impact on overall satisfaction(β=0.486,P<0.001).The overall satisfaction score of patients with medical insurance was 3.75±0.80.Among all dimensions,satisfaction with health service institutions was the highest,while satisfaction with medical insurance financing services was the lowest.Patients covered by urban employee basic medical insurance reported higher satisfaction with financing services.Employees of enterprises and institutions,as well as retirees,showed higher satisfaction with compensation and administrative services,whereas self-employed individuals and unemployed persons reported lower satisfaction.Patients with poorer self-rated health had lower overall satisfaction.Conclusions Medical staff and patients reported relatively high satisfaction with the"Double Package"payment method,though group differences existed.It is recommended to enhance medical staff satisfaction by optimizing the performance-based compensation system,strengthening technical empowerment,and improving policy training.For patients,efforts should focus on optimizing financing services and implementing targeted support for key groups(e.g.,flexibly employed persons,vulnerable health groups)to enhance their satisfaction with medical insurance.关键词
