Glycemic traits and dementia with Lewy body:Mendelian randomization study

Abstract
Objective To systematically evaluate the potential causal relationship between gly-caemic traits and the risk of dementia with Lewy bodies(DLB)from a genetic epidemiological per-spective using the Mendelian randomization(MR)method.Methods Based on publicly available genome-wide association study(GWAS)data,fasting glucose,glycated hemoglobin(HbA1c),fast-ing insulin,and type 2 diabetes were selected as exposure variables,and DLB as the outcome varia-ble.The inverse variance weighted(IVW)method was employed to infer the potential causal rela-tionship between glycaemic traits and DLB risk,supplemented by MR-Egger regression and MR-PRESSO analysis to detect horizontal pleiotropy and heterogeneity.Cochran's Q test was used to assess heterogeneity,and leave-one-outanalysis was conducted to verify the robustness of the results.Results IVW analysis indicated that both fasting glucose(OR=2.47,95%CI,1.14 to 5.38,P=0.02)and HbA1c levels(OR=1.31,95%CI,1.02 to 1.62,P=0.03)were positively causally associated with an increased risk of DLB.No significant associations were observed between genetic variants related to fasting insulin,type 2 diabetes and the risk of DLB.The sensitivity analysis results were consistent,and reverse MR analysis did not reveal significant reverse causal effects of DLB on gly-caemic indices.Conclusion This study provides genetic epidemiological evidence that hyperglycemic states increase the risk of DLB,suggesting that glycaemic control may be of great significance in the prevention strategies for DLB.关键词
