老年非小细胞肺癌化疗病人衰弱感知影响因素的质性研究
Influencing factors of preoperative frailty perception in elderly patients with non-small cell lung cancer undergoing chemotherapy:a qualitative study

Abstract
Objective:To explore the real experience of the influencing factors of frailty perception in elderly patients with non-small cell lung cancer(NSCLC)undergoing chemotherapy.Methods:A descriptive qualitative study was adopted.Sixteen elderly NSCLC patients with chemotherapy frailty were selected through purulent sampling for semi-structured in-depth interviews.Guided by the frailty mixed conceptual model,the interview data were analyzed using the directed content analysis method.Results:Four themes with 11 sub-themes were identified.1)Physical dimension:the coexistence of multiple diseases leaded to an increased burden of physical and mental symptoms,and the aging process caused progressive decline in the functions of various systems.2)Psychological dimension:negative emotions leaded to the continuous depletion of psychological resilience resources,debilitating and disastrous thinking weakened coping effectiveness,and alexithymia affected the process of emotion regulation.3)Social characteristic dimensions:the shrinking of social networks leaded to an increase in social mental health risks,economic burdens affected negative cognitive biases and internalization of anxiety,and family support falled into a predicament of emotional compromise and scientific intervention.4)Behavior dimension:sedentary lifestyle posed hidden risks to health management,polypharmacy behavior was correlated with drug safety problems and there was insufficient support for diverse nutritional intake information needs.Conclusions:The perception of frailty in elderly NSCLC patients undergoing chemotherapy is influenced by social,physical,psychological and behavioral factors.In view of this,it can provide a basis for evaluating,identifying and formulating personalized rehabilitation plans,thereby effectively improving the clinical outcomes of elderly NSCLC patients.关键词
