益气养阴清热解毒汤联合顺铂灌注治疗气阴两虚型肺癌恶性胸腔积液对患者免疫功能、炎症反应及肺纤维化的影响

Abstract
Objective To investigate the effects of the Yiqi Yangyin Qingre Jiedu decoction combined with cisplatin perfusion on the immune function,inflammatory response and pulmonary fibrosis in the treatment of patients with malignant pleural effusion(MPE)of lung cancer with Qi and Yin deficiency.Methods A total of 80 MPE patients admitted to the hospital from January 2020 to February 2025 were selected as the research subjects.They were divided into the control group(receiving thoracic puncture drainage followed by cisplatin perfusion treatment)and the observation group(receiving Yiqi Yangyin Qingre Jiedu decoction treatment on the basis of the control group),with 40 cases in each group.The pulmonary fibrosis and adverse reactions and immune function indicators[CD3+,CD4+,CD8+T cell proportions and CD4+/CD8+ratio],serum inflamma-tory factors[C-reactive protein(CRP),interleukin-6(IL-6),serum tumor necrosis factor-α(TNF-α)],pulmo-nary function indicators[forced vital capacity(FVC),first-second forced vital capacity(FEV1),maximum re-spiratory flow rate(PEF)],Traditional Chinese Medicine(TCM)Symptom scores were compared between the two groups before and after treatment.Results After 3 weeks of treatment,the CD3+T cell,CD4+T cell proportions and CD4+/CD8+ratio in the two groups were higher than those before treatment,and the CD3+T cells,CD4+T cell proportions and CD4+/CD8+ratio in the observation group were higher than those in the control group,with statistically significant differences(P＜0.05).After 3 weeks of treatment,the serum CRP,IL-6,TNF-α levels and CD8+T cell proportion in the two groups were lower than those before treat-ment,and the serum CRP,IL-6,TNF-α levels and CD8+T cell proportion in the observation group were lower than those in the control group,with statistically significant differences(P＜0.05).After 3 weeks of treat-ment,the FVC,FEV1 and PEF in the two groups were higher than those before treatment,and the FVC,FEV1 and PEF in the observation group were higher than those in the control group,with statistically significant differences(P＜0.05).After 3 weeks of treatment,the scores of cough with little phlegm,low-pitched cough,fatigue,hot flushes and night sweats,dizziness and blurred vision and pale complexion,as well as the total scores in the two groups were lower than those before treatment,and the scores of cough with little phlegm,low-pitched cough,fatigue,hot flushes and night sweats,dizziness and blurred vision,and pale complexion,as well as the total scores in the observation group were lower than those in the control group,with statistically significant differences(P＜0.05).After 3 weeks of treatment,the differences in pulmonary fibrosis grades be-tween the two groups were statistically significant(P＜0.05).During the treatment period,the incidence of bone marrow suppression and nausea and vomiting in the observation group was lower than that in the control group(P＜0.05).Conclusion The Yiqi Yangyin Qingre Jiedu decoction can improve the immune function of MPE patients,reduce inflammatory response,improve pulmonary function and reduce TCM Syndrome scores and the incidence of pulmonary fibrosis.关键词
