HIV感染合并肺癌患者的临床病理学特征及免疫状态分析
Clinicopathological characteristics and analysis of immune status of patients with HIV infection and pulmonary malignancies

Abstract
Objective To investigate the clinicopathological characteristics of patients with HIV infection and lung cancer and their relationship with peripheral immune status.Methods A retrospective analysis was performed on the clinicpathological data of 51 patients with HIV infection and lung cancer.Immunohistochemistry(IHC)was conducted to detect the expression of CK7,TTF-1,p40,Syn,Ki67 and other proteins in each group.Flow cytometry was used to measure peripheral blood CD4+and CD8+T-cell counts,and the CD4+/CD8+ratio was calculated to analyze its association with pathological subtypes and immunophenotypes.Results The main pathological types were adeno-carcinoma(56.9%)and squamous cell carcinoma(35.3%).The Ki67 proliferation index was significantly higher in squamous cell carcinoma than in adenocarcinoma(P<0.05),and the CD4+/CD8+ratio was significantly lower(median 0.39 vs 0.72,P<0.05).Patients with CK7 negative expression also showed a significantly lower CD4+/CD8+ratio compared to those with CK7 positivity(P<0.05).ROC curve analysis indicated that the CD4+/CD8+ratio had moder-ate diagnostic value for predicting squamous cell carcinoma(AUC=0.732 8).At the optimal cutoff value of 0.611 1,the sensitivity and specificity were 61.11%and 82.76%,respectively.Conclusion There appears to be an associa-tion between the pathological type and peripheral immune status in patients with HIV infection and lung cancer.The CD4+/CD8+ratio may serve as a potential indicator reflecting immune function and tumor subtype,suggesting its poten-tial clinical utility in the evaluation and differential diagnosis of HIV infection-associated lung cancer.关键词
