Treatment of early-stage steroid-induced osteonecrosis of the femoral head based on the theory of"Yang governing Yin to follow"
摘要
Abstract
Steroid-induced osteonecrosis of the femoral head(SONFH)is a well-recognized complication of glucocorticoid therapy.Its incidence has now surpassed that of traumatic osteonecrosis,drawing increasing attention from the medical community toward early prevention and treatment.In traditional Chinese medicine(TCM),the kidney governs bones and generates marrow.Yuan Yang of the kidney plays a leading role in bone formation and repair.The deficiency of Yuan Yang,exhaustion of marrow,impaired Qi transformation,and accumulation of turbid stasis in the bone are considered the key pathological mechanisms of SONFH.The theory of"Yang governing Yin to follow"originates from the Huangdi Neijing and is one of the essential perspectives in TCM's Yin-Yang theory.It emphasizes the leading role of Yang Qi within the dynamic balance of Yin and Yang.The governor vessel connects internally with the kidney and commands the Yang Qi of the entire body.To tonify the life-gate Yang Qi,moxibustion on the governor vessel is often recommended.Therefore,the treatment of SONFH should focus on warming the governor vessel to support Yang,supplemented by tonifying the kidney and replenishing marrow,along with locally activating Yang to resolve stasis and eliminate turbidity.Based on the theory of"Yang governing Yin to follow",this study discusses the etiology,pathogenesis,and characteristic external treatment methods for early-stage SONFH,aiming to provide new ideas and approaches for TCM treatment.关键词
