Abstract
Essential hypertension falls under the categories of"vertigo"and"headache"in traditional Chinese medicine(TCM).This disease has a high prevalence worldwide,with over 245 million affected individuals in China.Research indicates that TCM possesses unique value in the treatment of hypertension.Ancient medical practitioners primarily elucidated its etiology and pathogenesis from the perspectives of wind,phlegm,and deficiency.Originating from the Menghe medical school,Yan's internal medicine proposed that"disorder of the ascending and descending of Qi movement"is the key pathogenesis of hypertension.It suggests that the early stage of the disease is primarily characterized by hyperactivity of liver Yang and liver depression transforming into fire,while the middle and late stages often manifest as dual deficiency of Yin and Yang and deficiency-Yang disturbing upward.The treatment adheres to the principle of the"balance method(Heng Fa)",establishing three main therapeutic strategies:Descending Qi and fire,ascending the clear and descending the turbid,and tonifying the lung and kidney,with flexible modifications based on pattern differentiation.This paper explores the pathogenesis of disordered ascending and descending of Qi movement in Yan's internal medicine and the application of the methods of descending Qi and reducing fire in the treatment of hypertension.Additionally,it summarizes the unique insights of Yan's internal medicine in treating hypertension based on clinical experience.关键词
