基于方证诊治小儿食积新探
New Exploration of Diagnosis and Treatment of Children Food Accumulation Based on Formula-Syndrome Theory

Abstract
Food accumulation,a common pediatric digestive disorder,stems from"Spleen deficiency and stagnation in the middle energizer",characterized by pathological features such as qi stagnation,phlegm-dampness retention,heat transformation,and disrupted pivot mechanisms.Clinically,it manifests as the"three excesses",such as hyperactivity with sweating,frequent belching with phlegm,and abdominal pain with restlessness.Children's immature yin and yang and weak spleen-stomach pre-dispose them to undigested food accumulating in the gastrointestinal tract,often progressing to the heat stage,which is one of three phases of"accumulation,heat,and deficiency".Treatment focuses on unblocking stagnation through formulas that disperse ac-cumulation,harmonize shaoyang,and regulate the spleen-stomach.The approach prioritizes resolving accumulation over clearing heat,combining clearing,harmonizing,and purging methods.Dachaihu Decoction(大柴胡汤)from Treatise on Cold Damage Dis-eases employs classic couplet medicinals,such as Chaihu-Huangqin,Dahuang-Zhishi,Banxia-Shengjiang,and Shaoyao-Zh-ishi,in order to harmonize shaoyang,purge heat,and relieve stagnation,aligning with pediatric food accumulation patterns.There-fore,its modified use shows notable efficacy.Based on the formula-syndrome theory,the study analyzes the etiology,"three ex-cesses",core couplet medicinals of Dachaihu Decoction,and clinical indications,supported by a case example for practical guid-ance.关键词
