Abstract
Objective To investigate the relationship between serum circular RNA(circRNA)ACAP2 and circRNA CDR1as levels and the severity and prognosis of patients with ST-segment elevation myocardial infarction(STEMI).Methods A total of 190 STEMI patients admitted to the Affiliated Hospital of Hebei University of Engineering from January 2021 to November 2023 were enrolled as the study subjects.They were divided into a mild group with 98 cases and a severe group with 92 cases based on Gensini scores.According to different prognostic outcomes,patients were classified into a good prognosis group with 138 cases and a poor prognosis group with 52 cases.The levels of circRNA ACAP2 and circRNA CDR1as were detected using qRT-PCR.Pearson correlation analysis was used to examine the correlation between serum levels of circRNA ACAP2 and circRNA CDR1as.Spearman correlation analysis was applied to analyze the correlation between circRNA ACAP2 and circRNA CDR1as levels and Gensini scores.Logistic regression analysis was conducted to identify influencing factors for the prognosis in STEMI patients.ROC curve analysis was performed to evaluate the predictive value of serum circRNA ACAP2 and circRNA CDR1as levels for the prognosis of STEMI patients.Results The serum levels of circRNA ACAP2 and circRNA CDR1as in the severe group were significantly higher than those in the mild group(t=17.535,27.748,respectively;P<0.05).Pearson correlation analysis showed a positive correlation between serum levels of circRNA ACAP2 and circRNA CDR1as(P<0.05),and Spearman correlation analysis indicated that both serum circRNA ACAP2 and circRNA CDR1as levels were positively correlated with Gensini scores(P<0.05).The differences in onset time,Killip classification,number of stents implanted,and serum levels of circRNA ACAP2 and circRNA CDR1as between the good and poor prognosis groups were statistically significant(t/χ2=10.153,7.154,15.882,29.436,33.294,respectively;P<0.05).Multivariate Logistic regression analysis results indicated that onset time,Killip classification,number of stents implanted,circRNA ACAP2,and circRNA CDR1as were influencing factors for the prognosis of STEMI patients(P<0.05).ROC curve analysis showed that the AUCs for predicting the prognosis of STEMI patients using serum circRNA ACAP2 and circRNA CDR1as levels were 0.843 and 0.818,respectively.The combined prediction of the two indicators yielded an AUC of 0.900,which was better than the prediction of either marker alone(Z=2.563,2.601;P<0.05).Conclusion The serum levels of circRNA ACAP2 and circRNA CDR1as in STEMI patients are closely related to the severity and prognosis of the disease.Their combined detection has certain clinical value for predicting patient prognosis.关键词
