增生型糖尿病视网膜病变伴单眼玻璃体积血患者的虹膜荧光素血管造影影像特征
Imaging features of iris fluorescence angiography in patients with prolifera-tive diabetic retinopathy complicated by unilateral vitreous hemorrhage

Abstract
Objective To investigate the imaging features and significance of iris fluorescein angiography(IFA)in patients with proliferative diabetic retinopathy complicated by unilateral vitreous hemorrhage.Methods In this retrospec-tive case-control study,55 patients with proliferative diabetic retinopathy complicated by unilateral vitreous hemorrhage were enrolled,who were examined by fundus fluorescein angiography(FFA)combined with IFA preoperatively at Henan Provincial Eye Hospital between January 2021 and February 2026.Fifty-five affected eyes underwent vitrectomy combined with preoperative intravitreal injection.All affected eyes and contralateral eyes received best corrected visual acuity,intraoc-ular pressure,fundus photography,slit lamp microscopy,and IFA combined with FFA examinations before surgery.Accord-ing to IFA imaging results,they were divided into a diabetic iridopathy group(12 patients)and a non-diabetic iridopathy group(43 patients).Fisher's exact probability method was used to compare the detection rate of diabetic iridopathy by slit lamp microscopy and IFA,and the independent samples t-test,paired samples t-test,or Pearson's x2 test was used to com-pare the data between the two groups.Results IFA examination revealed that 21.8％(12/55)of the affected eyes and 20.0％(11/55)of the contralateral eyes in the 55 patients had diabetic iridopathy.FFA examination showed that all 55 con-tralateral eyes had proliferative diabetic retinopathy.Among the 12 eyes with diabetic iridopathy,IFA detected non-prolifera-tive diabetic iridopathy in 7 eyes and proliferative diabetic iridopathy in 5 eyes;slit-lamp microscopy detected proliferative diabetic iridopathy in 3 eyes.The difference in detection rates between the two methods was statistically significant(P＜0.001).Among the contralateral eyes of the 12 patients with diabetic iridopathy,IFA examination showed diabetic iridopathy in 11 eyes,including non-proliferative diabetic iridopathy in 8 eyes and proliferative diabetic iridopathy in 3 eyes;slit-lamp microscopy detected only 2 eyes with proliferative diabetic iridopathy.The difference in detection rates between the two methods was statistically significant(P＜0.001).Conclusion IFA can detect whether patients with proliferative diabetic retinopathy and vitreous hemorrhage have concurrent diabetic iridopathy before surgery,facilitating individualized treatment.关键词
