MRI-based quantitative evaluation of vascular distribution in different regions of the anterior cruciate ligament

Abstract
Objective To explore the clinical application value of contrast-enhanced quantitative magnetic resonance imaging(qMRI)in evaluating the perfusion characteristics in different regions of the human anterior cruciate ligament(ACL).Methods The contrast-enhanced qMRI data of the knee joints in 42 patients were retrospectively analyzed.According to the pre-contrast oblique sagittal images,the entire length of the ACL was equally divided into three regions:proximal,middle and distal regions(each accounting for 1/3 of the total length).The relative percentage of perfusion in each region to the total ACL perfusion was calculated by quantifying the degree of MRI signal enhancement.Results There were statistically significant differences in the relative perfusion among the proximal,middle and distal regions of the ACL(P<0.05).The relative perfusion of the proximal region was significantly higher than that of the middle(P=0.007)and distal regions(P=0.001),while there was no statistically significant difference in relative perfusion between the middle and distal regions(P=0.281).Subgroup analysis showed that the relative perfusion of the proximal region of the ACL in males was higher than that in females,with a statistically significant difference(P=0.036).Conclusion Based on contrast-enhanced qMRI,this study confirmed that the vascular distribution of the ACL is mainly located in the proximal 1/3 region,accounting for more than half of the total perfusion.Clarifying the regional differences of ACL vascular distribution may provide an important imaging theoretical basis for understanding the pathogenesis of ACL injury and optimizing surgical strategies for ACL repair and reconstruction.关键词
