Effect of nutritional scoring system based on hemoglobin,albumin,lymphocytes and platelets on prognosis of patients with multiple myeloma

Abstract
Objective To analyze the prognostic value of the nutritional score composed of hemoglobin,albumin,lymphocytes,and platelets(HALP score)in patients with multiple myeloma(MM).Methods The clinical data of 107 newly diagnosed MM patients in the Second Hospital of Tianjin Medical University from December 2020 to December 2024 were collected.The HALP score was calculated based on the levels of hemoglobin,albumin,lymphocyte count,and platelet count in peripheral blood.ROC curve analysis was used to calculate the optimal cutoff value of HALP scores.Kaplan-Meier method was used to draw survival curves.Log-rank test and COX regression analysis were used to explore possible factors affecting patient prognosis.Results The optimal cut-off value with the ROC curves for the HALP score was 20.8.The patients were divided into two groups according to the optimal value of the HALP score[Low-score group:HALP＜20.8(n=49)and high-score group HALP≥20.8(n=58)].The OS and PFS of the high HALP score group were higher than those of the low HALP score group(P=0.003,P=0.005).Univariate analysis showed thatage＞60,β 2-microglobulin＞5.0 mg/L,ISS stage(Ⅲ),and low HALP score(HALP＜20.8)were prognostic risk factors for OS in multiple myeloma patients(P=0.001,P=0.031,P=0.021,P=0.003).Multivariate Cox regression analysis revealed that age＞60,low HALP score(HALP＜20.8)and ISS stage(Ⅲ)were independent risk factors for OS in MM patients(P=0.005,P=0.008,P=0.021).Conclusion The H ALP score may be a valuable prognostic indicator in multiple myelomapatients.关键词
