初诊伴淋巴瘤相关性贫血弥漫大B细胞淋巴瘤患者临床特征及预后分析
Clinical characteristics and survival analysis of newly diagnosed diffuse large B-cell lymphoma with lymphoma-related anemia

Abstract
Objective To explore the clinical characteristics and prognosis of newly diagnosed diffuse large B-cell lymphoma with lymphoma-related anemia.Methods A retrospective analysis was performed on the newly diagnosed patients with non-primary central nervous system(non-pCNS)DLBCL in the Second Affiliated Hospital of Soochow University from January 2014 to June 2023,and the patients were divided into groups according to the Hemoglobin level and anemia of different causes.Results From January 2014 to June 2023,218 patients with non-pCNS DLBCL were newly diagnosed in our hospital,62(28.4％)of them were complicated by lymphoma-related anemia,with 36 males,and their mean hemoglobin is(107.9±16.5)g/L(normal range 130-175 g/L);and the number of females is 26,mean hemoglobin is(97.5±12.3)g/L(normal range 115-150 g/L);Compared with patients in the non-anemia group,patients of lymphoma-related anemia group have higher tumor load(high LDH andβ 2-GM)and were easier to have risk factors such as B symptoms,advanced Ann Arbor,intermediate and high risk of IPI score,and early recurrence.Survival analysis suggested that:DLBCL with lymphoma-related anemia have undifferentiated OS and PFS compared with patients with anemia of different causes,and are related to the degree of anemia;comparing with DLBCL patients who have normal hemoglobin,PFS is poorer(P＜0.05),but have undifferentiated OS.Conclusion Patients with early diagnosed DLBCL with lymphoma-related anemia have high tumor load and many risk factors of poorer prognosis,with poorer early prognosis.Clinicians should identify them in the early diagnosis and pay attention to their possible long-term prognosis.关键词
