超声引导下富血小板血浆液压扩张联合手法松解治疗冻结肩的效果
Efficacy of Ultrasound-guided Hydraulic Dilation with Platelet-rich Plasma and Manual Release in the Treatment of Frozen Shoulder

Abstract
Objective:To evaluate the clinical efficacy of ultrasound-guided hydraulic dilation with platelet-rich plasma(PRP)combined with manual release in the treatment of frozen shoulder.Method:A total of 80 patients with frozen shoulder treated at Department of Orthopedics,Qinhuangdao First Hospital between April 2022 and April 2024 were selected and divided into PRP group and conventional group using a random number table method.Among them,40 patients in PRP group received ultrasound-guided hydraulic dilation with PRP combined with manual release,while the other 40 patients in conventional group received traditional drug injection combined with manual release.The VAS scores of shoulder pain at rest and during movement,the shoulder Constant-Murley score(CMS)before treatment and at 1 week,1 month,and 3 months after treatment,complication rate,clinical efficacy(the excellent and good rate)3 months after treatment and recurrence rate within 1 year of follow-up were compared between the two groups.Result:Shoulder VAS scores for pain at rest and during movement and CMS significantly improved at all time points after treatment in both groups compared to before treatment(P<0.05).At 1 week,1 month,and 3 months after treatment,the shoulder VAS scores for pain at rest and during movement,as well as the shoulder CMS were significantly better in the PRP group than in the conventional group(P<0.05).At 3 months after treatment,the excellent and good rate in the PRP group(92.5％)was significantly higher than that in the conventional group(75.0％)(P<0.05).The recurrence rate within 1 year was significantly lower in the PRP group(5.0％)than in the conventional group(22.5％)(P<0.05).Conclusion:Ultrasound-guided hydraulic dilation with PRP combined with manual release is a safe and effective treatment for frozen shoulder.This therapy combined with manual release can significantly alleviate pain,accelerate the recovery of shoulder joint function,and reduce the risk of recurrence.关键词
