疏肝解郁安神汤联合重复经颅磁刺激治疗帕金森病合并失眠(肝气郁结型)患者的临床效果
Clinical Effect of Shugan Jieyu Anshen Decoction Combined with Repetitive Transcranial Magnetic Stimulation in the Treatment of Patients with Parkinson's Disease Complicated with Insomnia(Liver Qi Stagnation Type)

Abstract
Objective:To explore the clinical effect of Shugan Jieyu Anshen Decoction combined with repetitive transcranial magnetic stimulation(rTMS)in the treatment of patients with Parkinson's disease(PD)complicated with insomnia(liver Qi stagnation type).Method:A total of 80 patients with PD complicated with insomnia(liver Qi stagnation type)from September 2024 to July 2025 who were admitted to Ezhou Hospital of Traditional Chinese Medicine were selected and divided into the study group(n=40)and the control group(n=40)according to the random number table method.The control group received rTMS treatment,and the study group received Shugan Jieyu Anshen Decoction combined with rTMS treatment.Both groups were treated for one month.The clinical efficacy,TCM syndromes scores,sleep quality,PD symptoms,polysomnography parameters,and neurotrophic factors indicators in both groups were compared.Result:The total effective rate of the study group after 1 month of treatment was 95.00%(38/40),which was higher than 77.50%(31/40)of the control group(P<0.05).After one month of treatment,the scores of TCM syndromes,Parkinson's disease sleep scale-2(PDSS-2),and unified Parkinson's disease rating scale(UPDRS)in both groups were lower than those before treatment,and those in the study group were lower than those in the control group(P<0.05).After one month of treatment,sleep latency(SL)in both groups were shortened compared to before treatment,sleep efficiency(SE)were increased compared to before treatment,total sleep time(TST)were prolonged compared to before treatment,and awakening times(AT)were reduced compared to before treatment,and compared to the control group,SL in the study group shorter,SE higher,TST longer,and AT fewer(P<0.05).After one month of treatment,the levels of 5-hydroxytryptamine(5-HT)and brain-derived neurotrophic factor(BDNF)in both groups were higher than those before treatment,and those in the study group were higher than those in the control group(P<0.05).Conclusion:The combination of Shugan Jieyu Anshen Decoction and rTMS in the treatment of patients with PD complicated with insomnia(liver Qi stagnation type)can improve clinical effect,alleviate clinical symptoms,improve sleep quality,and regulate the levels of neurochemical factors.关键词
