Abstract
Objective:To explore the correlation between inflammatory factors,the neutrophil-to-lymphocyte ratio(NLR)level and the prognosis of patients with multiple myeloma(MM).Method:The clinical data of 120 patients with MM in Jiujiang NO.1 People's Hospital from March 2019 to March 2022 were retrospectively selected,and the patients were followed up for 3 years.According to the survival situation,they were divided into the survival group(78 cases)and the death group(42 cases).The levels of CRP,IL-6,TNF-α,neutrophils and lymphocytes were detected within 24 hours after admission for all patients.The influencing factors of prognosis of MM patients were analyzed,and the ROC curve was used to analyze the predictive efficacy of inflammatory factors and NLR for the prognosis of MM patients.Result:The age,proportion of international staging system(ISS)stage Ⅲ,and levels of CRP,IL-6,TNF-α and NLR in the death group were higher than those in the survival group(P<0.05).logistic regression analysis showed that increasing age[OR=5.087,95%CI(1.497,17.283)],ISS stage Ⅲ[OR=2.643,95%CI(1.613,4.331)],high IL-6 level[OR=3.061,95%CI(1.696,5.522)],high TNF-α level[OR=2.474,95%CI(1.585,3.860)],high NLR level[OR=3.258,95%CI(1.650,6.432)]were independent risk factors for the death of MM patients(P<0.05).ROC curve showed that the AUC of IL-6,TNF-α,NLR and the combination of the three factors for predicting prognosis of MM patients were 0.797,0.807,0.775 and 0.920,respectively.Conclusion:In addition to the traditional independent risk factors of increased age and ISS stage Ⅲ,high levels of IL-6,TNF-α and NLR in peripheral blood are independent risk factors for the death of MM patients,and the combined detection of these three indicators has a good value in predicting the prognosis of MM patients.关键词
