基于智能前置审方系统的门诊不合理用药情况分析
Analysis of Irrational Drug Use in Outpatient Clinics Based on Intelligent Pre-Examination Prescription System

Abstract
Objective To analyze the interception of irrational drug use by the pre-examination prescription system to promote rational clinical medication use.Methods The outpatient prescriptions in a tertiary hospital from May 1,2024 to April 30,2025 were retrospectively analyzed.The irrational drug use intercepted by the pre-examination prescription system(PASS Clinical Pharmacy Management System V3.0)and the intervention situation pushed by the Hospital Information System(HIS)were counted and analyzed.Results A total of 5 994 662 orders were reviewed by the pre-examination prescription system,1 722 851 irrational orders(28.74%)were intercepted,mainly included excessive daily dosage(608 859 orders,35.34%),off-label indication(574 892 orders,33.37%),and dosage range exceeding(163 179 orders,9.47%).The top 10 irrational orders were intercepted by the departments of Rheumatology and Immunology(215 283 orders,12.50%),Nephrology(212 234 orders,12.32%),Dermatology and Venereology(112 253 orders,6.52%),Cardiology(96 971 orders,5.62%),Urology(86 736 orders,5.03%),General Practice(VIP)Ward(71 351 orders,4.14%),Gastroenterology(63 451 orders,3.68%),Tumor Daycare(62 300 orders,3.62%),Integrated Traditional Chinese and Western Medicine(61 877 orders,3.59%),and Hematology(54 893 orders,3.19%).The number of prescription intervention entries pushed in the HIS by pharmacists showed a trend of first significantly reduced,then relatively stable,and finally fluctuating slightly.Conclusion The pre-examination prescription system can effectively reduce clinical irrational drug use,but there are still situations such as false positives and false negatives.It is necessary to establish a feedback mechanism in the prescription review work and continuously optimize the system rules to improve the level of rational drug use.关键词
