摘要

Abstract

Guided by the theory of"kidney fragility",this study posits that inherent kidney fragility or the loss of renal dynamic equilibrium constitutes the core internal factor in the pathogenesis of diabetic kidney disease(DKD)."Kidney Fragility"encompasses not only generalized kidney deficiency but also emphasizes the inherent vulnerability and imbalance in the kidney's structural and functional microenvironment.This fragility predisposes the kidneys to greater susceptibility,earlier onset,and more severe damage under comparable pathological insults.The pathogenesis initiates with"stagnation of the dual Yang"triggering"kidney fragility",followed by the downward flow of internal heat and turbid toxins to the lower burner.This process progressively evolves into essence depletion,Qi exhaustion,and collateral impairment,ultimately leading to the formation of concretions within the kidney collaterals.In clinical practice,individualized treatment strategies should be formulated based on the distinct pathological characteristics at different disease stages and the specific constitutional types of kidney fragility,thereby providing theoretical guidance for the precision prevention and management of DKD.
