Abstract
Objective:To investigate the effects of different doses of bortezomib combined with chemotherapy on minimal residual disease(MRD)negativity rate and long-term survival in patients with multiple myeloma(MM).Methods:A total of 120 MM patients who received two different doses of bortezomib combined with chemotherapy in our hospital from September 2019 to September 2024 were retrospectively selected and divided into conventional dose group(bortezomib 1.3 mg/m2 combined with chemotherapy)and low-dose group(bortezomib 1.0 mg/m2 combined with chemotherapy),with 60 cases in each group.Clinical efficacy, inflammatory indicators[C-reactive protein(CRP),tumor necrosis factor-α(TNF-α),interleukin-6(IL-6),interleukin-17A(IL-17A)],renal function indicators[serum creatinine(SCr),β2-microglobulin(β2-MG),blood urea nitrogen(BUN),uric acid(UA)]and adverse reactions were compared between the two groups. All patients were followed up for 15 to 75 months, and MRD negative rate and long-term survival were counted. Results:There was no significant difference in objective response rate between low-dose group and conventional dose group(81.67%vs 86.67%,P>0.05).After 4 courses of treatment,CRP,TNF-α,IL-6,IL-17A,SCr,β2-MG,BUN and UA in both groups were decreased(P<0.05),but there was no significant difference between the two groups(P>0.05).There were no significant differences in the incidence of infection, peripheral neuropathy, gastrointestinal symptoms, thrombocytopenia and leukopenia between the two groups(P>0.05).There were no significant differences in MRD negative rate and mortality rate between low-dose group and conventional dose group(78.33%vs 71.67%,10.00%vs 15.00%,P>0.05).Conclusion:Bortezomib 1.0 mg/m2 and 1.3 mg/m2 combined with chemotherapy have comparable efficacy and safety in the treatment of MM,with no significant differences in MRD negative rate and survival prognosis.关键词
