Association between the triglyceride-glucose index and pericoronary adipose tissue inflammation

Abstract
Objective To explone the direct association between the triglyceride-glucose(TyG)index a marker of insulin resistance,and the fat attenuation index(FAI),a novel imaging biomarker of coronary inflammation,in patients with type 2 diabetes mellitus(T2DM).The aim is to establish an early warning system for coronary inflammation based on the metabolic-inflammation axis.Methods This study enrolled 428 T2DM patients who underwent coronary computed tomography angiography(CCTA)between October 2023 and November 2024.Patients were stratified into three groups according to the tertiles of the TyG levels.The correlation between TyG levels and multi-vessel coronary artery disease was analyzed,and the distribution characteristics were visually presented.After adjusting for confounding factors,linear regression analysis was applied to elucidate the quantitative relationship between TyG and FAI.Results This study demonstrates a significant positive correlation between the TyG index and the FAI of multiple coronary arteries(P<0.05)in a T2DM population.Notably,the association between perivascular tissue attenuation and the TyG index was strongest for the right coronary artery(RCA).Quantitative analysis revealed that for each standard deviation increase in the TyG index,the RCA-FAI significantly increased by 3.40 units(P<0.01).Conclusion This study systematically demonstrates,in a T2DM population,a significant association between the TyG index and FAI of multiple coronary arteries,with the strongest correlation observed for RCA-FAI,thereby providing a critical foundation for early cardiovascular risk identification and precise intervention.关键词
