慢性乙型肝炎肝纤维化患者中医证型与血清铁代谢的相关性研究
Study on the correlation between different TCM syndromes and serum iron metabolism indicators in patients with chronic hepatitis B hepatic fibrosis

Abstract
Objective:The purpose of this study is to explore the relationship between serum iron metabolism indicators and different traditional Chinese medicine(TCM)syndromes in patients with hepatic fibrosis due to chronic hepatitis B(CHB),which may provides clinical evidence for the research on the mechanisms of TCM syndromes.Methods:A cross-sectional study was conducted,collecting cases from 100 patients with CHB-related hepatic fibrosis.General information,examination results(four indicators of hepatic fibrosis,liver transient elastography,etc.),and TCM syndrome information were collected from the patients.Serum samples were collected to measure the expression levels of serum ferritin(SF),transferrin(TRF),and hepcidin(Hepc).Results:There were significant differences in liver stiffness measurement(LSM),laminin(LN),procollagen type Ⅲ N-terminal peptide(PⅢNP),type Ⅳ collagen(CⅣ),hyaluronic acid(HA),SF,and TRF levels among patients with different TCM syndromes of CHB-related liver fibrosis.SF was positively correlated with PⅢNP,HA,and CⅣ,while TRF was negatively correlated with the four indicators of hepatic fibrosis.Multivariate regression analysis showed that TRF was associated with yin deficiency of liver and kidney,yang deficiency of spleen and kidney,damp-heat in liver and gallbladder,and liver qi stagnation with spleen deficiency.Conclusion:The distribution of TCM syndromes in patients with CHB-related hepatic fibrosis to some extent reflects the evolution process of TCM pathogenesis.The changes in serum SF and TRF levels are highly consistent with the evolution of TCM pathogenesis in different TCM syndromes,which may reflect part of the molecular biological mechanisms of different TCM syndromes in hepatic fibrosis.关键词
