Abstract
Objective:To investigate the association between preoperative serum levels of caspase-1,leptin,and macrophage inflammatory protein-3α(MIP-3α)and the incidence of acute liver function deterioration(ALFD)following transarterial chemoembolization(TACE)in hepatocellular carcinoma(HCC)patients,and to evaluate their predictive utility for ALFD.Methods:A total of 129 patients with intermediate-to-advanced HCC who underwent TACE at our hospital between June 2020 and May 2024 were enrolled.Serum caspase-1,leptin,and MIP-3α levels were measured before TACE,and the occurrence of ALFD was recorded post-TACE.Multivariate logistic regression analysis was performed to identify independent risk factors for ALFD,while receiver operating characteristic(ROC)curves were used to assess the predictive accuracy of these biomarkers.Results:ALFD developed in 35 patients(27.1%)after TACE.Compared with the non-ALFD group,the ALFD group exhibited significantly higher baseline levels of caspase-1,leptin,and MIP-3α(P<0.05).Independent risk factors for ALFD included baseline Child-Pugh grade B,portal vein tumor thrombosis,and elevated caspase-1,leptin,and MIP-3α levels(P<0.05).The areas under the curve for predicting ALFD after TACE in liver cancer patients based on caspase-1,Leptin,MIP-3α,preoperative Child-Pugh classification,and the presence of portal vein tumor thrombus were 0.781,0.770,0.810,0.665,and 0.410 respectively.The combined prediction area was 0.912,which was higher than the individual predictions(P<0.05).Conclusion:Elevated pretreatment serum levels of caspase-1,leptin,and MIP-3α are closely associated with post-TACE ALFD in HCC patients.The combined use of these biomarkers provides superior predictive performance for ALFD risk,offering valuable clinical insights for pretreatment risk stratification and personalized management.关键词
