中药封包联合抗结核药方案治疗结核性腹膜炎疗效及其对炎症介质水平的影响观察
Clinical efficacy of Chinese medicine envelope therapy combined with anti-tuberculosis drug regimen in the treatment of tuberculous peritonitis and its effects on the level of inflammatory mediators

Abstract
Objective To investigate the clinical efficacy of Chinese medicine envelope therapy combined with anti-tuberculosis regimen in patients with tuberculous peritonitis(TBP)and its impact on inflammatory mediator levels.Methods A total of 170 TBP patients admitted to Shaanxi Provincial Tuberculosis Prevention and Control Hospital from April 2022 to May 2023 were selected and randomly divided into a control group and an observation group,with 85 cases in each group,using the envelope method.The control group received basic treatment including anti-tuberculosis drug therapy,abdominal paracentesis for ascites drainage,and nutritional support.The observation group received Chinese medicine envelope adjuvant therapy in addition to the treatment given to the control group.Inflammatory mediator levels[interferon-γ(IFN-γ),tumor necrosis factor-α(TNF-α),interleukin-6(IL-6),and soluble interleukin-2 receptor(sIL-2R)],number of abdominal paracenteses,ascites absorption time,peritoneal thickness,absorption of tuberculous lesions,clinical efficacy,and adverse reactions were compared between the two groups before and after treatment.Results The clinical efficacy in the observation group was superior to that in the control group(P<0.05).After treatment,the levels of IFN-γ,TNF-α,IL-6,and sIL-2R in both groups were significantly lower than those before treatment(P<0.05),and were lower in the observation group than in the control group(P<0.05).Compared with the control group,the observation group had fewer paracenteses and shorter ascites absorption time(P<0.05).There was no significant difference in peritoneal thickness between the two groups before treatment(P>0.05),while after treatment,peritoneal thickness in the observation group was lower than that in the control group(P<0.05).The effective absorption rate of tuberculous lesions in the observation group was higher than that in the control group(P<0.05),and the incidence of adverse reactions was lower in the observation group than in the control group(P<0.05).Conclusion The combination of Chinese medicine envelope therapy with a conventional anti-tuberculosis regimen in TBP patients can improve clinical efficacy,reduce inflammatory mediator levels,alleviate clinical symptoms,and decrease the incidence of adverse reactions.关键词
