电针夹脊穴联合中药治疗气阴两虚兼血瘀型Wagner 0级糖尿病足的临床观察
Clinical efficacy of electroacupuncture at Jiaji points combined with Chinese herbal medicine for treating Wagner grade 0 diabetic foot with syndrome of qi-yin deficiency and blood stasis

Abstract
Objective To observe the clinical efficacy of electroacupuncture at Jiaji points combined with Chinese herbal medicine in treating Wagner grade 0 diabetic foot(DF)with syndrome of qi-yin deficiency and blood stasis.Methods Sixty patients with Wagner grade 0 DF who visited the First Department of Peripheral Vascular Disease at The First Affiliated Hospital of Heilongjiang University of Chinese Medicine from June to December 2024 were selected and randomly divided into an observation group and a control group,with 30 cases in each group,using a stratified block randomization method.The control group was treated with intravenous alprostadil and safflower yellow pigment for injection,combined with oral Chinese herbal decoction.The observation group received additional electroacupuncture at Jiaji points on the basis of the control group's treatment.Both groups were treated for 14 days as one course.Clinical efficacy,symptom scores before and after treatment,dorsal foot hemodynamic indicators(dorsalis pedis artery diameter,peak flow velocity,blood flow volume),ankle-brachial index(ABI),and hemorheological indicators[whole blood viscosity(high-shear and low-shear),plasma viscosity(PV),and fibrinogen(FIB)],as well as treatment safety,were compared between the two groups.Results The total effective rate in the observation group was higher than that in the control group(P<0.05).After treatment,all symptom scores in both groups were significantly decreased compared with those before treatment(P<0.05),and were lower in the observation group than in the control group(P<0.05).Peak flow velocity,blood flow volume,and ABI in both groups were increased after treatment compared with baseline(P<0.05),and were higher in the observation group than in the control group(P<0.05).No statistically significant differences were found in vascular diameter before and after treatment in either group(P>0.05).Whole blood viscosity,PV,and FIB were decreased after treatment in both groups(P<0.05),and were lower in the observation group than in the control group(P<0.05).No abnormalities were observed in blood routine,urine routine,or liver and kidney function before and after treatment in either group,and no serious adverse reactions occurred during treatment.Conclusion Electroacupuncture at Jiaji points combined with Chinese herbal medicine shows favorable efficacy in treating Wagner grade 0 DF with qi-yin deficiency and blood stasis.It can significantly improve clinical symptoms and signs and has regulatory effects on both hemodynamic and hemorheological indicators.关键词
