EASIX在初诊非M3型急性髓细胞性白血病化疗中的预后判断价值
The prognostic value of EASIX in the chemotherapy treatment of newly diagnosed non-M3 type acute myeloid leukemia

Abstract
Objective To evaluate the prognostic value of endothelial activation and stress index(EAS-IX)in patients with newly diagnosed non-M3 type acute myeloid leukemia undergoing chemotherapy treat-ment.Methods A retrospective cohort study design was adopted,including all newly diagnosed non-M3 type AML patients who received chemotherapy at this center from 2014 to 2023.EASIX was calculated as lactate dehydrogenase(U/L)×SCr(mg/dL)/PLT(×109/L).Patients were divided into the low EASIX group(EASIX≤40.585)and the high EASIX group(EASIX>40.585)based on the third quartile of EASIX.The differences in clinical characteristics,treatment regimens,and prognosis between the two groups were com-pared.Survival curves were constructed using Kaplan-Meier,and univariate and multivariate analyses were performed using the Cox proportional hazards regression model.Results The overall mortality rate(93.22%vs.82.58%)and the early mortality rate(13.56%vs.5.61%)of patients in the high EASIX group after treatment were both higher than those in the low EASIX group(P<0.05).Survival analysis showed that the overall survival(OS)time and disease-free survival(DFS)time of patients in the high EASIX group were shorter(P<0.05).Multivariate Cox regression analysis results indicated that EASIX was an independent prognostic factor for OS time,but it did not show predictive ability for DFS time.Internal validation using a 7∶3 grouping and 5-fold cross-validation showed that the receiver operating characteristic(ROC)curve of the prognostic model based on EASIX had good area under the curve(AUC)and good calibration,and the deci-sion curve analysis(DCA)confirmed its reliable clinical practical value.Conclusion EASIX can serve as an effective indicator for predicting the prognosis of patients with non-M3 type AML undergoing chemotherapy treatment at the initial stage.关键词
