PD-1抑制剂联合放化疗治疗局部晚期鼻咽癌的有效性和安全性研究
Study on the efficacy and safety of PD-1 inhibitor combined with chemoradiotherapy in the treatment of locally advanced nasopharyngeal carcinoma

Abstract
Objective To investigate the efficacy and safety of programmed cell death-1(PD-1)inhibi-tors combined with radiotherapy and chemotherapy in locally advanced nasopharyngeal carcinoma.Methods A retrospective analysis was conducted on the clinical data of 86 patients with locally advanced na-sopharyngeal carcinoma who were admitted to Cancer Hospital Affiliated to Guizhou Medical University from April 2019 to June 2022.Survival analysis was performed using the Kaplan-Meier method,and the Cox propor-tional hazards model was used to explore the association between various clinical factors and prognosis.Results The median overall survival(OS)time,progression-free survival(PFS)time,and distant metastasis-free survival(DMFS)time for all patients were 63,58,and 63 months respectively.The 3-year OS rate,PFS rate,and DMFS rate were 90.8%,90.0%,and 90.8%respectively.The results of multivariate Cox regression analysis showed that before treatment,EB virus(EBV)DNA(HR=5.404,95%CI:1.218-23.976),the ra-diotherapy dose of the primary lesion(HR=0.231,95%CI:0.075-0.707),and the radiotherapy dose of lymph nodes(HR=0.231,95%CI:0.075-0.707)were independent influencing factors for the OS time of patients with locally advanced nasopharyngeal carcinoma after PD-1 inhibitor combined with radiotherapy and chemotherapy(P<0.05).Conclusion The combination of PD-1 inhibitors with radiotherapy and chemother-apy shows excellent short-term efficacy in locally advanced nasopharyngeal carcinoma,and the overall adverse reactions are tolerable.关键词
