急性脑梗死患者血清lncRNA-Safe和SFRP2表达与颈动脉粥样硬化斑块的相关性
Correlation between the expressions of serum lncRNA-Safe and SFRP2 and carotid atherosclerotic plaque in patients with acute cerebral infarction

Abstract
Objective To study the correlation between serum lncRNA-Safe,SFRP2 levels and carotid atherosclerosis(CAS)and plaque stability in patients with cerebral infarction.Methods Eighty-four stroke patients admitted to Harbin 242 Hospital between March 2022 and March 2024 were included as subjects,the patients were divided into the plaque free group(24 cases)and the plaque group(60 cases)based on the presence or absence of concomitant CAS plaques,and according to the stability of plaques,they were divided into the stable plaque group(24 cases)and the unstable plaque group(36 cases).The expression level of lncRNA-Safe in serum was detected by qRT-PCR,the expression of serum SFRP2 was detected by enzyme linked immunosorbent assay(ELISA).Pearson method was used to analyze the correlation between lncRNA-Safe and SFRP2 expressions.The diagnostic value of lncRNA-Safe and SFRP2 for cerebral infarction with CAS plaques and plaque properties was evaluated using receiver operating characteristic(ROC)curves.Logistic regression was used to analyze the factors affecting the stability of CAS plaques.Results Compared with the plaque free group,the proportion of hypertension,the levels of TC,LDL-C,serum lncRNA-Safe and SFRP2 increased obviously in the plaque group(P<0.05).The levels of TC,lncRNA-Safe and SFRP2 in the unstable plaque group were obviously higher than those in stable plaque group,and the level of HDL-C was obviously lower than that in the stable plaque group(P<0.05).Pearson analysis showed that serum lncRNA-Safe and SFRP2 were positively correlated in patients with cerebral infarction in the plaque group(r=0.324,P=0.012).ROC analysis showed that the AUC of serum lncRNA-Safe,SFRP2,and their combined diagnosis of CAS plaque comorbidity were 0.824,0.769,and 0.894,respectively.The diagnostic efficacy of their combined detection was higher than that of SFRP2 alone,but the difference was not statistically significant compared to lncRNA-Safe alone(Zcombined-lncRNA-Safe=1.307,Zcombined-SFRP2=2.565,P=0.191,0.010).The AUC of serum lncRNA-Safe,SFRP2,and their combined evaluation of CAS plaque instability were 0.808,0.801,and 0.870,respectively.The efficacy of their combined evaluation was not statistically significant compared to the individual evaluation of serum lncRNA-Safe and SFRP2(Zcombined-lncRNA-Safe=1.515,Zcombined-SFRP2=1.731,P=0.130,0.083).Logistic analysis showed that TC,HDL-C,lncRNA-Safe and SFRP2 all had effect on plaque stability(P<0.05).Conclusion The levels of serum lncRNA-Safe and SFRP2 in patients with cerebral infarction complicated with carotid atherosclerotic plaque are obviously increased,and both of them have a certain impact on plaque stability.The combination of the two has a good evaluation value on carotid atherosclerotic plaque and plaque stability.关键词
