MRI峰度扩散成像在糖尿病周围神经病变中的应用
Application of MRI DKI in peripheral neuropathy of type 2 diabetes

Abstract
Objective To investigate the application value of MRI kurtosis diffusion imaging(DKI)in type 2 diabetic peripheral neuropathy(DPN).Methods MRI DKI technique was used to scan the knee joint of 16 patients with type 2 diabetic peripheral neuropathy(DPN group)and 9 patients without peripheral neuropathy(CG group)who were hospitalized at the same time.DKI related parameters of volume of interest(VOI)at different sites on sciatic nerve,tibial nerve and peroneal nerve of the two groups were obtained,including mean kurtosis(MK),axial kurtosis(AK),radial kurtosis(RK),mean diffusivity(MD),axial diffusivity(AD),radial diffusivity(RD)and fractional anisotropy(FA)based on DTI model,and these indicators were analyzed.Results Analysis of variance showed no statistically significant differences in MK,RK,AK,MD,FA,RD,and AD at the 4 sites between the CG group and the DPN group(P>0.05).In the DPN group,the DKI-derived kurtosis metrics MK,RK,and AK at the 4 sites of the sciatic nerve,tibial nerve,and peroneal nerve were significantly lower than those in the CG group(P<0.05),with the difference being more pronounced in MK.No statistically significant differences were observed in FA,MD,AD,or RD(P>0.05).Conclusion DKI technique can identify early microstructural changes in nerves associated with DPN.Among its parameters,MK,AK,and RK are more sensitive than traditional DTI parameters and hold promise as imaging biomarkers for the early diagnosis of DPN.关键词
