Analysis of traditional Chinese medicine constitution distribution and clinical characteristics in neu-romyelitis optica spectrum disorder
摘要
Abstract
Objective This study aims to systematically analyze the distribution characteristics of traditional Chinese medicine(TCM)constitutions in patients with neuromyelitis optica spectrum disorder(NMOSD)and to preliminarily explore the relationship between different constitution types and clinical characteristics. Methods Using a cross-sectional survey method,92 NMOSD patients were enrolled.Clinical data were collected, and constitution information was obtained through the'TCM Constitution Classification and Determination'questionnaire.Correlation analysis and cluster analysis were conducted on biased constitutions. The distribution patterns of TCM constitutions in NMOSD were studied by grouping according to clinical syndromes, age, and disease duration. Results Among the 92 NMOSD patients, biased constitutions accounted for90.90%,and mixed constitutions accounted for75.00%.The top three constitutions were Yang-deficiency(41.30%),Qi-deficiency(10.87%),and Phlegm-dampness(10.87%).In the overall constitution analysis,Qi-deficiency and Yang-deficiency were the most prevalent, accounting for 57.61%and 68.48%respectively,making them the most common constitution types in NMOSD patients.Significant correlations were found between different constitution types and clinical characteristics(P<0.05).Specifically,limb weakness was associated with Qi-stagnation;cold limbs with Yang-deficiency;vertigo with Blood-stasis;cognitive decline with Blood-stasis and Qi-stagnation;and constipation with Yang-deficiency and Qi-stagnation.Correlation analysis among biased constitutions in NMOSD showed strong positive correlations between Qi-deficiency and Yang-deficiency,Qi-deficiency and Qi-stagnation,Qi-stagnation and Blood-stasis,as well as Phlegm-dampness and Damp-heat(P<0.01).Cluster analysis indicated that Qi-deficiency,Yang-deficiency,Qi-stagnation,Phlegm-dampness,and Damp-heat were closely related and tended to co-occur.There were no statistically significant differences in the distribution of TCM constitutions among patients with different initial clinical syndromes, age groups,or disease durations(P>0.05).Conclusion The TCM constitution of NMOSD patients is predominantly characterized by biased constitutions, often manifesting as mixed types.Among them,Qi-deficiency and Yang-deficiency are the primary biased constitution types. The analysis of TCM constitutions and clinical characteristics suggests that different constitution types determine the tendency of clinical symptoms during the pathological evolution of the disease. In NMOSD,Qi-deficiency,Yang-deficiency,Qi-stagnation,Phlegm-dampness,and Damp-heat are closely interrelated and prone to co-oc-currence.关键词
