肺泡灌洗液靶向二代测序技术检测在疑似肺结核患者的诊断价值
Diagnostic value of bronchoalveolar lavage fluid targeted next-generation sequencing testing in patients with suspected pulmonary tuberculosis

Abstract
Objective To investigate the early diagnostic value of bronchoalveolar lavage fluid Targeted next-generation se-quencing(tNGS)testing in patients with suspected pulmonary tuberculosis(PTB).Methods A total of 320 patients with sus-pected PTB admitted to Hami Central Hospital from April 2024 to February 2025 were selected as study subjects.All patients underwent tNGS testing,Xpert testing,sputum smear testing,and Mycobacterium tuberculosis(MTB)culture testing on bronchoalveolar lavage fluid.The diagnostic efficacy of the four testing methods for PTB were compared using the clinical diagnosis results as the reference standard.Results Among the 320 patients with suspected PTB,175 were confirmed as PTB and 145 as non-PTB.The Kappa values of tNGS of bronchoalveolar lavage fluid,Xpert MTB/RIF assay,sputum smear examination,and MTB culture were 0.875,0.772,0.335,and 0.478,respectively.The sensitivity of tNGS of bronchoalveolar lavage fluid was higher than that of Xpert MTB/RIF assay,sputum smear examination,and MTB culture,with statistically significant differences(P＜0.001).Consistency analysis showed that tNGS had good consistency.Conclusion Bronchoalve-olar lavage fluid tNGS testing has good diagnostic performance for PTB,which can reduce the rate of missed diagnosis,promote early treatment,and interrupt the transmission chain.关键词
