Bidirectional effect of pain expression inhibition behavior and parental anxiety level in preschool children with fractures:A study based on EMA

Abstract
Objective:To investigate the bidirectional effect of pain expression inhibition behavior and parental anxi-ety level in preschool children with fractures based on ecological momentary assessment(EMA).Methods:A prospec-tive cohort study will be conducted from January 2024 to March 2025.100 preschool children with fractures and their parents were selected,and their pain expression behavior through facial expression observation and changes in heart rate(HR)and respiratory rate(RR)were evaluated,while the parentswere evaluated for anxiety levels using the Hamilton Anxiety Scale-14(HAMA-14),all once a day for one week.The multi-layer linear model was used to analyze the correlation between pain expression in pediatric patients and parental anxiety levels.Results:The pain scores,HR and RR of the child,and parental anxiety scores showed trends of first increasing and then decreasing,all reaching their highest point on the 4th day.The gender(t=45.638,5.344,59.835,5.692;P＜0.05),age(t=15.754,7.216,8.840,7.409;P＜0.05),pain score(t=12.229,5.766,15.612,6.075;P＜0.05),HR(t=9.152,5.179,11.495,5.105;P＜0.05),and RR(t=5.893,5.214,6.258,5.160;P＜0.05)of the affected child could all affect the level of pa-rental anxiety.And the gender(t=8.722,5.193;P＜0.05),age(t=7.331,4.080;P＜0.05),father anxiety score(t=9.992,6.147;P＜0.05),and mother anxiety score(t=9.122,6.185;P＜0.05)of the affected child could all affect pain score,and the gender(t=6.429,5.481,5.170,3.980;P＜0.05),age(t=4.566,4.814,4.329,6.114;P＜0.05),father anxiety score(t=7.956,7.753,5.564,7.554;P＜0.05),and mother anxiety score(t=8.393,8.036,9.064,9.930;P＜0.05)of the affected child could all affect HR,and RR.Conclusion:The pain,HR,and RR of preschool children with fractures first increase and then decrease,and the trends of parental anxiety levels are basically consistent with those,and the two can interact with each other.关键词
