基于老年综合评估的门诊患者睡眠障碍潜在剖面分析与自理能力的关系研究
Latent profile analysis of sleep disorders and its association with self-care ability in elderly outpatients:a study based on comprehensive geriatric assessment

Abstract
Objective To explore the potential sleep categories and their relation with self-care ability in elderly outpatients.Methods From September to December 2024,elderly outpatients were selected using convenience sampling at a tertiary grade-A hospital in Shiyan,Hubei.Based on comprehensive geriatric assessment standards,patients were surveyed with a General Information Question-naire,the Athens Insomnia Scale,Self-rating Anxiety Scale,the Activities of Daily Living Scale,the Wong-Baker Faces Pain Rating Scale,and the Frailty Assessment Scale.Potential profile analysis was performed based on Athens Insomnia Scale scores;binary logis-tic regression was conducted for independent factors for different categories,and association rule analysis for joint influencing factors.Results This study included 360 elderly outpatients,and they were divided into two groups:those with relatively good sleep(59.4%,214/360)and those with difficulty falling asleep and insufficient sleep(40.6%,146/360).Logistic regression revealed that heart disease(OR=5.518,P<0.001),nocturia(OR=2.699,P=0.009),hypnotic use(OR=3.695,P<0.001),anxiety(OR=3.752,P=0.001),and self-care ability(OR=5.224,P<0.001)were independent influencing factors for sleep disorder.Association rule analysis indicated that the com-bined presence of hypnotic use,diabetes,and hypertension were co-occurring factors for the difficulty falling asleep and insufficient sleep(support 6.39%,confidence 95.65%,lift 2.36).Conclusion Sleep in elderly outpatients differs significantly.Difficulty in falling asleep and insufficient sleep are closely related to self-care ability.Medical staff should focus on elderly outpatients with difficulty falling asleep and insufficient sleep,and take precise measures to improve their sleep quality and self-care ability to promote healthy aging.关键词
