Mediating Effect of Hope Level on Stigma and Quality of Life in Stroke Patients with Deglutition Disorders

Abstract
Objective To explore the mediating effect of hope level on stigma and quality of life in stroke patients with deglutition disorders.Methods Convenience sampling method was used to select stroke patients with deglutition disorders who were hospitalized at Liuzhou Traditional Chinese Medicine Hospital from January 2024 to August 2025 as the research subjects.The General Information Questionnaire,Stigma Scale for Stroke Patients(SSS),Chinese version of Herth Hope Index(HHI),and Chinese version of Swallowing-related Quality of Life(SWAL-QOL)quality of life dimension were used to investigate the general information,stigma,hope level,and quality of life of patients.The correlation analysis of SSS,Chinese version of HHI,and Chinese version of SWAL-QOL quality of life dimension scores in stroke patients with deglutition disorders was conducted using Pearson correlation analysis;the Model 4 module in the Process program was used to verify the mediating effect of hope level on stigma and quality of life in stroke patients with deglutition disorders.Results There were statistically significant differences in Chinese version of SWAL-QOL quality of life dimension scores among the stroke patients with deglutition disorders with different ages,marital status,Wada drinking water test grading,and whether indwelling gastric tube(P<0.05).Pearson correlation analysis results showed that the SSS scores of stroke patients with deglutition disorder were negatively correlated with the Chinese version of HHI scores(r=-0.559,P<0.01)and the Chinese version of SWAL-QOL quality of life dimension scores(r=-0.448,P<0.01),while the Chinese version of HHI scores were positively correlated with the Chinese version of SWAL-QOL quality of life dimension scores(r=0.577,P<0.01).The mediation effect model showed that the level of hope played a partial mediating effect on stigma and quality of life in stroke patients with deglutition disorders(the mediation value was-0.886,and accounted for 59.3％of the total effect).Conclusion The stigma in stroke patients with deglutition disorders is negatively correlated with the level of hope and quality of life,while the level of hope is positively correlated with the quality of life;and the level of hope plays a partial mediating effect on stigma and quality of life in stroke patients with deglutition disorders.关键词
