糖化血红蛋白、胆囊收缩素及胰高血糖素样肽1水平与妊娠期糖尿病合并糖尿病胃轻瘫的相关性及其预测价值
Relationship of the levels of glycosylated hemoglobin A1c,cholecystokinin and glucagon-like peptide 1 with diabetic gastroparesis in gestational diabetes mellitus patients and their predictive value

Abstract
Objective:To investigate the relationship of the levels of glycosylated hemoglobin A1c(HbA1c),cholecystokinin(CCK),and glucagon-like peptide 1(GLP-1)with diabetic gastroplegia(DGP)in gestational diabetes mellitus(GDM)patients and their predictive value.Methods:A total of 100 GDM patients with suspected DGP admitted to the Affiliated Hospital of Anhui Health College from Jan 2022 to Dec 2024 were selected as the research subjects.They were divided into DGP group(n=47)and non-DGP group(n=53).The gastrointestinal symptoms were evaluated using the Gastrointestinal Symptom Rating Scale(GSRS),and the levels of HbA1c,CCK and GLP-1 were detected.Pearson correlation analysis was used to examine the relationships among the the variables.Multivariate logistic regression analysis was performed to identify the influencing factors of GDM combined with DGP.Receiver operating characteristic(ROC)curve was used to evaluate the diagnostic value of HbA1c,CCK,and GLP-1 for DGP in GDM patient.Results:The DGP group had significantly higher levels of HbA1c and GLP-1 and a significantly lower level of CCK compared to the non-DGP group(P＜0.05).Pearson correlation analysis showed that GSRS score was positively correlated with HbA1c and GLP-1(r=0.328,0.363),and negatively correlated with CCK(r=-0.419)(P＜0.05).Multivariate logistic regression analysis showed that elevated HbA1c(OR=1.857,95%CI:1.191-2.896)and elevated GLP-1(OR=1.506,95%CI:1.074-2.111)were independent risk factors for DGP in GDM patients,while elevated CCK(OR=0.747,95%CI:0.616-0.906)was a protective factor(P＜0.05).The area under the ROC curve for the combined prediction of HbA1c,CCK,and GLP-1 was 0.863,with a sensitivity of 83.00%and a specificity of 81.10%.Conclusions:Patients with GDM complicated by DGP exhibit abnormal expression levels of HbA1c,CCK,and GLP-1.The combination of these indicators has high predictive value for DGP in GDM patients.关键词
