Abstract
Objective To investigate the predictive efficacy of combined detection of pre-treatment CT cerebral perfusion imaging parameters for short-term prognosis of elderly patients with acute cerebral infarction(ACI).Methods 138 elderly patients with ACI admitted to Lushan County People's Hospital from October 2022 to September 2024 were selected.According to the prognosis at 90 days after treatment,the patients were divided into the poor prognosis group(n=43)and the good prognosis group(n=95).The CT cerebral perfusion imaging parameters[cerebral blood volume(CBV),cerebral blood flow(CBF),mean transit time(MTT)and time to peak(TTP)],and NIHSS score were compared between the two groups.The correlations between each parameter and the admission NIHSS score were analyzed,and the predictive efficacy of CT cerebral perfusion imaging parameters for the short-term prognosis of patients was analyzed.Results Compared with the good prognosis group,the poor prognosis group had lower CBV and CBF,longer MTT and TTP,and higher NIHSS score(P<0.05).Pearson correlation analysis showed that CBV and CBF were negatively correlated with NIHSS score,while MTT and TTP were positively correlated with NIHSS score(P<0.05).Logistic regression analysis showed that CBV,CBF,MTT and TTP were all independent influencing factors of short-term prognosis(P<0.05).The area under the curve(AUC)of combined detection of CBV,CBF,MTT and TTP for predicting short-term prognosis was 0.927,higher than that of single parameter prediction(P<0.05).Conclusions The parameters measured by pre-treatment CT cerebral perfusion imaging(CBV,CBF,MTT,TTP)are significantly associated with the disease severity and short-term prognosis of elderly ACI patients.The combined assessment of multiple parameters has certain predictive efficacy for the short-term outcomes of patients,and can provide a reference basis for the development of clinical intervention strategies.关键词
