苏州市社区老年人健康素养现状及影响因素分析:基于生活质量视角
Analysis of the Current Situation and Influencing Factors of Health Literacy among the Elderly in Communities of Suzhou City Based on the Perspective of Life Quality

Abstract
Objective To analyze the health literacy level of the elderly in Suzhou communities and its influencing factors from the perspective of quality of life,and provide a theoretical basis for formulating targeted health literacy promotion strategies.Methods A two-stage probability proportional to size sampling method was used to select 637 elderly people aged 65 and above from 23 communities of Suzhou City.A general information questionnaire,SF-12 scale,and the Chinese Residents'Health Literacy Monitoring Questionnaire were used for the investigation.Logistic regression model was used to analyze the influencing factors of health literacy level of community elderly in different quality of life groups,and random forest model was used to rank the importance of influencing factors.Results The health literacy level of the elderly in the communities of Suzhou City is 29.3％.Their knowledge and concept literacy,lifestyle and be-havior literacy,and skill literacy accounted for 33.3％,28.9％and 64.2％,respectively.The multivariate Logistic regression analysis showed that educational level,occupation,alcohol consumption,sleep disorders,annual medical expenses,and self-assessed health status were the influencing factors of the health literacy of the elderly in the communities of Suzhou City.The prediction accuracy of the test set of low physical health(low PCS),high physical health(high PCS),low mental health(low MCS)and high mental health(high MCS)groups of the random forest model was between 0.737 and 0.782,the accuracy rate was between 0.714 and 0.755,and the AUC was between 0.730 and 0.802.The model performance was good.The top two influencing factors of health literacy in different quality of life groups were education level and occupation.In addition,the low PCS group was also affected by drinking and sleep disor-ders,and the high PCS group was affected by the cost of medical treatment.The low MCS group was affected by self-rated health sta-tus,and the high MCS group was affected by drinking and self-rated health status.Conclusion The overall health literacy level of the elderly is relatively low,in which the health literacy of the physical health group is significantly higher than that of the mental health group.Secondly,the common influencing factors of health literacy among different groups of elderly people in terms of quality of life mainly include educational level and occupation,but the intensity of their effects varies among different groups.The heterogeneous influ-encing factors include alcohol consumption,sleep,self-assessed health status,and disease burden.It is recommended that in prac-tice,based on the characteristics of the elderly population's quality of life and the group differences in the influencing factors of health literacy,stratified and categorized health education and intervention plans can be formulated.关键词
