Abstract
[Objective]To investigate the correlation between serum levels of 5-hydroxyindoleacetic acid(5-HIAA),interleukin-1β(IL-1β),and neuropeptide Y(NPY)and sleep quality in patients with sleep disorders after cerebral infarction, and to analyze their predictive value for the occurrence of sleep disorders after cerebral infarction.[Methods]A prospective study was conducted on 120 patients with cerebral infarction diagnosed and treated in Anyang Third People's Hospital from December 2022 to December 2024,serving as the study group.Additionally,120 healthy individuals who underwent physical examinations during the same period were selected as the control group.Patients were categorized into two subgroups based on whether sleep disorders occurred three months after cerebral infarction:the occurrence subgroup and the non-occurrence subgroup. The levels of serum 5-HIAA,IL-1β,and NPY were compared across the groups.Sleep quality parameters, including total sleep time(TST),sleep latency(SL),sleep efficiency(SE),rapid eye movement sleep(REMS)time, and arousal index(AI),were analyzed for both the occurrence and non-occurrence subgroups.Furthermore, the correlation between serum levels of 5-HIAA,IL-1β,and NPY and sleep quality was examined.A logistic regression analysis was performed to investigate the relationship between serum levels of 5-HIAA,IL-1β,and NPY and sleep disorders after cerebral infarction. The predictive value of serum levels of 5-HIAA,IL-1β,and NPY for sleep disorders after cerebral infarction was evaluated.[Results]The levels of serum 5-HIAA and NPY in the study group were lower than those in the control group, while the level of IL-1β was higher(P<0.05).The incidence of sleep disorders in 120 patients with cerebral infarction was 37.50%(45/120).In the subgroup with sleep disorders,TST,SE,and REMS were lower than those in the subgroup without sleep disorders, while SL and AI were higher(P<0.05).The age,NIHSS score at admission, proportion of alcohol consumption history, and serum IL-1β level one month after treatment in the subgroup with sleep disorders were higher than those in the subgroup without sleep disorders. The levels of serum 5-HIAA and NPY one month after treatment were lower than those in the subgroup without sleep disorders(P<0.05).5-HIAA and NPY were positively correlated with TST and REMS,but negatively correlated with SL(P<0.05).IL-1β was negatively correlated with TST and REMS,but positively correlated with SL(P<0.05).After adjusting for age,National Institutes of Health Stroke Scale(NIHSS)score at admission, and alcohol consumption history, serum IL-1β level one month after treatment was an independent risk factor for sleep disorders after cerebral infarction, while serum 5-HIAA and NPY levels one month after treatment were independent protective factors for sleep disorders after cerebral infarction(P<0.05).The area under the receiver operating characteristic curve(AUC)value of serum 5-HIAA,IL-1β,and NPY combined for predicting sleep disorders after cerebral infarction was significantly higher than that of individual indicators(P<0.05).[Conclusion]Patients with sleep disorders after cerebral infarction exhibit elevated serum levels of IL-1β and reduced levels of 5-HIAA and NPY,all of which are closely correlated with sleep disturbances and sleep quality post-onset.The combined detection of these levels demonstrates superior predictive efficacy for the occurrence of sleep disorders after cerebral infarction compared with individual indicators.关键词
