Abstract

Objective:To explore key causes of the lag that the system identifies alarm,and optimize management strategies for equipment,and ensure the quality of platelets of apheresis,and the safety of donors,which aimed at a series of problems include the insufficient amount of collecting platelet,the missed judgment for adverse reactions in donors due to the lag of the alarm system of equipment of collecting platelet.Methods:The data about operation of the Trima Accel blood cell separator(TerumoBCT,USA)at the Hebei Province Blood Center from January 2023 to January 2024 were selected.The Fault Tree Analysis(FTA)and Failure Mode and Effects Analysis(FMEA)were comprehensively applied to identify the key causes of occurring lag of alarms,and a"three-level troubleshooting-closed-loop verification"management strategy was constructed.Five blood cell separators were selected as the research subjects.From January to June 2024,the conventional management strategy was adopted,while the"three-level troubleshooting-closed-loop verification"hierarchical troubleshooting strategy was implemented during June and December 2024.The response efficiency of the alarm for fault of equipment,the quality and safety of collecting platelet were compared under the two modes.Results:The lagged time of alarm for equipment was(21.25±6.35)s,and the false alarm rate was(8.03±0.58)%after implementing hierarchical troubleshooting strategy,which were lower than(58.25±6.35)s and(24.04±1.25)%after implementing conventional management strategy,and the differences were significant(t=9.364,6.254,P<0.05).The rework rate of collection,and the adverse reaction rate of donors of implementing hierarchical troubleshooting strategy were significantly lower than those of implementing conventional management strategy,and the qualified rate of platelet products of implementing hierarchical troubleshooting strategy were significantly higher than that of implementing conventional management strategy,and the differences of them were significant(t=5.154,6.255,5.787,P<0.05).Conclusion:The"three-level troubleshooting-closed-loop verification"strategy can effectively solve the problem of lagged alarms for equipment of collecting platelet,which can provide reference for rational and efficient management for equipment of apheresis to platelet.
